DANCE HAVEN LIABILITY WAIVER & INTAKE FOR CHILDREN AND TEENS 

I hereby agree to the following: My child has permission to participate in a creative movement class at with The Dance Haven. My child is participating in classes during which my child will receive information and instruction in creative movement. I recognize that dance requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my child’s participation in any physical fitness program, including dance. I represent and warrant that my child has no medical condition that would prevent her/his/their participation in physical fitness activities. In consideration of being permitted to participate in the dance classes, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which my child might incur as a result of participating in the program. In further consideration of being permitted to participate in the dance classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries or damages that my child may sustain as a result of participating in classes or workshops  taught by The Dance Haven.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. 

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby grant The Dance Haven permission to use photos and videos of me/my child in any and all of its publications, including but not limited to all The Dance Haven’s printed and digital publications, social media accounts, and business advertisements. 

I acknowledge that since my participation with The Dance Haven is voluntary, I will receive no financial compensation.

I hereby irrevocably authorize The Dance Haven to edit, alter, copy, exhibit, publish or distribute photos of myself/my child for purposes of publicizing The Dance Haven's program or for any other related, lawful purposes. I waive the right to inspect or approve the finished product, including written or electronic  copies in which my/my child’s likeness appears. I waive any right to royalties or other compensation arising out of or related to the use of the photograph.

I am at least 18 years of age and competent to contract in my own name or I am the guardian of a participant who is under the age of 18. I have read this release before signing below with the intent of it being legally binding and fully understanding the contents, meaning and impact of this release.

*Important note: Dance instructors/assistants will not feed, assist in toileting, or give medication to any dancer. If you/your dancer needs assistance with any of these, please plan to have a support person present during the class. There are waiting areas right outside the dance rooms that you are welcome to use.

I understand that by entering my name below, I am in agreement with the above waiver and the media release option I chosen, and that by signing my name and submitting below, this is my digital signature.
